Video Sitemap Errors

Whenever I try to submit my Video Sitemap to Google it gets denied!!!

Errors say duplicate titles, descriptions, urls. My website consists only of pages with videos.
Why does my regular sitemap have no issues but the video sitemap does?
Video sitemap is the only reason I purchased this and it has not worked properly yet.

Video sitemap is more restrictive in this, make sure that all videos on your site have different titles/descriptions to avoid that.
